Dosage
Adults and adolescents: 5 mg/kg infused over 1 hour, every 8 hours for 5-10 days depending on condition.

For herpes simplex encephalitis, adults: 10 mg/kg every 8 hours for 10 days.
Adjust doses for patients with renal impairment as needed.
