Xarelto is a medication used to prevent blood clots and reduce the risk of stroke in people with certain heart conditions, as well as to treat conditions like deep vein thrombosis and pulmonary embolism.   • Reduce risk of major thrombotic vascular events in peripheral artery disease.
  • Treatment of venous thromboembolism in pediatric patients.
  • Thromboprophylaxis in pediatric patients with congenital heart disease after Fontan procedure.

Dosage

15 mg twice daily for 21 days, then 20 mg once daily for DVT/PE. 10 mg once daily for DVT prophylaxis after hip or knee surgery. 10 mg once daily for venous thromboembolism prophylaxis in acutely ill patients. 2.5 mg twice daily with aspirin for CAD or PAD.
Pediatric dosing varies by weight and age.
